The Health Status Indicator Electronic Data Order Form, can be used to order copies of the data used to produce this report. A number of files are available for single years 1994 through 1998, and aggregated for years 1994-1998. The file format descriptions provided indicate the field columns, length, and information stored in that field. In each file, data for the diagnostic group requested is aggregated by these fields.

In death data, the "all causes of death" file includes data for each major diagnostic and all sub-diagnostic codes. The files by major diagnosis include data for the relevant sub-diagnoses. Readers wanting to order data are advised to contact the Information and Resource Management Branch of the Vital Statistics Agency at the telephone or fax numbers shown below.
