If you are changing your own name or the name of your minor child 18 years of age, you will be required to complete a Change of Name Application form. Applications may also be obtained in person from one of our service provider locations listed under:

Telephone requests may be made by calling the British Columbia Vital Statistics Agency at 250-952-2681 to order the application form package.

Please note: We are unable to include the fingerprint kit for downloading from our Internet site as specialized envelopes, specifically developed for fingerprint filing and distribution, must be used and cannot be printed from our Internet site. You can obtain fingerprint kits in person, by phone or e-mail as per the information listed above.
