As of March 31, 2008, faxed applications will no longer be accepted.
Application forms are in Adobe Acrobat PDF (Portable Document Format) format that can be downloaded and printed. All other forms are available as printable PDF files only.
Please note that effective February 12, 2007, the Vital Statistics Agency has implemented centralized printing of all certificates from the Victoria office. All certificates will be mailed or couriered to clients direct from Victoria.
